Requisition Number 09-3747 Title Case Manager RN (Home Health) - Full Time City Towson State MD Description A Registered Nurse responsible for the care coordination and provision of quality home nursing care and maintaining the highest standards of care and ensuring compliance with all policies, procedures and regulatory requirements. Requirements 1. Current and unencumbered state license to practice as a Registered nurse. 2. At least one year experience as a Registered Nurse. One of those years must consist of full-time experience in providing direct patient care in the home health setting. One year supervisory/management experience preferred. 3. Current CPR certification 4. Able to assess patient status and identify requirements relative to age specific needs. 5.
