Ensures patient safety and satisfaction through optimal and medically appropriate utilization of clinical resources. Ensures appropriate reimbursement from third party payors. POSITION IS TEMPORARY. Responsibilities: The Case Manager will: Enhance communications among health team members. Establish partnerships with physicians and other care providers. Streamline clinical and administrative processes. Manage clinical resources appropriately. Achieve interdisciplinary integration for care delivery within the hospital, within the PHCS, and with external facilities and agencies. Increase patient, physician, and third party payor satisfaction. Identify and implement criteria including standards for inpatient length of stay and resource utilization guidelines. Arrange for services to patients in a seamless manner. Access and broker services to connect patients with community services. Track outcomes and identify areas for improvement. Enhance clinical documentation to improve delivery of patient care and capture the intensity of the care and services provided. Qualifications: RN with a current New Jersey license, BSN degree preferred.Minimum of 3 years Medical-Surgical experience required. Case Management and Discharge Planning experience required.
