DEPARTMENT: Outpatient Services
REPORTS TO: Associate Director Adult Outpatient
Brief outline of position:
1. Determine proper modality of treatment and provide appropriate therapy for each assigned client in accordance with his/her treatment plan.
2. Develop individual treatment plan for each assigned client in conjunction with Psychiatrist, Case Manager, and other members of the treatment team.
3. Serve as Primary Case Manager for all non-aftercare clients in caseload. This involves coordination of service delivery with other departments and agencies, consultation with treating Psychiatrist regarding medication compliance, and other issues, etc.
4. Provide crisis intervention services on an “on-call” basis. Duties include: conducting emergency evaluations, determining proper disposition and arranging for needed services for emergency cases, both walk-ins and active cases.
5. Conduct evaluations which include completion of the Comprehensive Biopsychosocial Evaluation/Reevaluation and Page 3 of the Client Registration Document (MH History, diagnosis, level of functioning, etc.)
6. Maintain the documentation in record/chart of clients in assigned caseload that is necessary to meet CBH and State Regulatory Standards. This includes, but is not limited to, regular review and update of treatment plans, daily progress notes that are consistent with treatment goals.
7. Submit all documentation pertaining to the billing for treatment services in a timely fashion, and assure that correct and updated information regarding insurance coverage is available.
8. Attend scheduled staff and supervision meetings
9. Participate in In-Service Training, Conferences, and Workshops as assigned and approved by Associate Director.
10. Perform other duties as assigned by supervisor
MINIMUM REQUIREMENTS: PhD. or Master’s Degree in Counseling, Sociology, Psychology or Social Work, and experience providing Outpatient Therapy &/or partial hospital settings required. Previous experience in a community mental health setting as well as knowledge of Mental Health Recovery approach approach is preferred. Must be able to work in a team milieu with other professionals who have diverse backgrounds and approaches. Must have ability to: A) Climb stairs; B) Assist with consumer restraint in emergency situations.
