Job Details: - Experience is required The Case Manager-Supportive Housing provides comprehensive, coordinated supportive services, in a case management method of service delivery, to recently homeless families in order to help them to remain in permanent housing and move toward self-sufficiency. EDUCATION/WORK EXPERIENCE: Must possess a degree in a human services field such as Social Work or Psychology, or be within one (1) year of receipt of degree. Must possess two (2) years experience in social services with low-income urban families. Must possess work organization and problem-solving skills; counseling skills; knowledge of community resources for the homeless; computer skills: Microsoft Word and other applications as required by Mercy Medical Center and grantor agencies. Must be able to perform case management of homeless families; assessment of homeless families for housing; facilitation of educational and support groups and documentation skills as required by grants.
