Social Worker - MSW Department: Care Management Schedule: Full Time Shift: Days Hours: 8:30am - 5pm Job Details: * Care Managementis a collaborative and facilitative process of business, interpersonal, and clinical strategies that, when successfully applied, effects more efficient delivery of care, reduces variations in the consumption of clinical resources, and produces improvement in clinical and financial outcomes. The Care Management Department Social Worker atProvidenceHospitalparticipates as an integral member of the Care Management Department, whichincludes the Physician Advisor, RN Care Managers, and support staff.Their focus is the patients social, financial, and discharge planning needs in support of the achievement of departmental and hospital quality and fiscal outcomes. Social Work cases are managed on a referral basis, both formal with a written order and informal as identified by the Social Worker themselves or from a member of the healthcare team. Referral sources include the RN Care Manager, the Physician, and/or the Clinical Staff and referrals reasons are as defined in the scope of this role description. The Providence Hospital Social Worker possesses excellent interpersonal skills, is team-oriented, and works efficiently to effect favorable patient satisfaction and discharge outcomes. Minimum Requirements: DC Licensure as LISCW by the Sate Board of Social Work Examiners Masters Degree in social Work form approved School of social Work and LGSW. LGSW,Minimum of one (1) years experience in an acute care health setting or related field. * Knowledge of clinical, legal, financial and general admission requirements for transfer to post acute care. * Knowledge of Medicare and Medicaid coverage for post acute care services. * Knowledge regarding Advanced Directives, competency and guardianship. * Knowledge about child and adult abuse and neglect dynamics and reporting requirements as well as the ability to assess and report accurately and implement safe and appropriate discharges. * Knowledge of psychosocial and legal issues related to domestic violence as well as community resources available to assist victims. * Knowledge of community resources, especially those for substance abuse treatment and homelessness. * Knowledge necessary to understand the patients plan of care. * Awareness of cultural issues and how it may impact patient interaction within the health care system. * Demonstrates organizational abilities and skill in setting priorities. Able to function effectively with little supervision. Take initiative in performing required duties. * Demonstrates the ability to utilize the e-Discharge system. * Demonstrates the ability to interact positively/constructively as a department and team member. * Demonstrates appropriate judgment and decision-making abilities. * Demonstrates the knowledge and skills necessary to provide services as defined in the job responsibilities and performance standards in this job description for all "Age Specific Patient Groups." * Possesses the ability to work closely with Patient Access, Business Office, Health Information Management, and Payers in a coordinated fashion to effect positive patient outcomes. * Possesses broad understanding of 3rdparty reimbursement. * Demonstrates skills coordinating multiple details to achieve a timely conclusion.
