Department Marketing Statement Founded in 1978, VNSNYS Long Term Home Health Care Program (LTHHCP) is the oldest program of its kind in the New York metropolitan area, as well as the largest in the US. The services we provide enable patients with chronic illness or disabilities to live at home instead of in a nursing home facility. These services include skilled nursing care, medical social work, nutrition counseling, rehabilitation services, and home health aide services, and are provided to patients throughout Brooklyn, the Bronx, Manhattan, and Queens. We are seeking talent for the opportunity described below. Responsible for providing social work services to patients and families in collaboration with the health care team consistent with VNS Home Care policies. Works under general supervision. Qualifications Licensure: Current Registration to practice as a Licensed Social Worker in New York State required. Valid drivers license may be required, as determined by operational/regional needs.Education: Masters Degree in Social Work after successfully completing a prescribed course of study at a graduate school of social work accredited by the Council on Social Work and the Education Dept. and who is licensed or certified by the Education Dept to practice social work in the State of New York, required. Experience: Minimum of one year experience as a social worker experience in a health care setting required. Bilingual skills may be required, as determined by operational needs.
