Under the direction of a Senior Social Worker and/or Supervisor, or his/her designee, provide Article 31 Mental Health services to children and adolescent patients and their families using various social work methods, including individual, group and family counseling. These functions are performed under general supervision, necessitating the ability to exercise independent and sound judgment. Illustrative Examples of Work*: Participate with interdisciplinary team members in developing and implementing program policies and procedures and preventing crises. Assess the social and emotional needs of patients. Provide direct counseling/social services to students referred by self, health team, faculty, administration and other school ancillary staff. Track and follow up on all referrals made in and out of school. Coordinate social work services and patient referrals between the program and other agencies providing social and educational services in both the school and local community. Establish referral network/linkages with outside agencies for both students and parents. Participate and occasionally present at teacher meetings, assemblies, etc. Serve as an advocate for patients rights within the clinic and serve as a mediator between parents, child and school. Remain available for crisis intervention social work as the need arises or arrange for such coverage by emergency social services. Record professional assessments, identify problems, treatment plans and progress notes as required by NYSOMH regulations, policies and procedures, and the Childrens Aid Society. Attend all weekly clinical meetings (e.g. Utilization Reviews, Disposition, Administrative). Ensure that all patients, visitors, guests, as well as co-workers, receive personalized, prompt attention and are treated with receptiveness, dignity and respect, no matter how limited the contact. Operate the on-call emergency phone on a rotating basis. Implement philosophy and objectives of program. Perform other related duties, as required.
