St. Clare Outreach - Social Worker II - Part Time Days

Job Summary:

As a member of a multi-disciplinary team, this job is responsible for assessing and providing support and intervention for the social and emotional needs of patients and their families to promote continuity of care and services following hospitalization.  Work involves professional activities such as psycho-social assessment, mental health evaluation and referral, crisis intervention, environmental assistance, patient advocacy, after-care planning and others of similar scope and complexity, depending on the assigned work unit.  Work conducted at this level requires a Masters degree in Social Work.

Essential Duties:

* Performs assessment and evaluation, focusing on the psychosocial needs of the patient and family; facilitates family conferences as appropriate.

* Interprets social, emotional and family stress to members of the care team as it relates to patients problems and plan of care.

* Plans and coordinates interventions; communicates plan to interdisciplinary patient care team and evaluates effectiveness of interventions for identified patients and families.

* Advocates on behalf of patients and caregivers for service access, and for the protection of the patients health, safety and rights.  In addition, advocates on behalf of the organization to promote safety, quality and appropriate resource utilization.

* Completes documentation responsibilities as identified by department policy and regulatory agencies.

* Provides consultation to staff and physicians on patient issues and provides protocol clarification/guidance.

* Develops and maintains professional relationships with medical staff, clinical team, community providers and payers to promote effective coordination of care across the continuum of services.

* Attends and actively participates in committees as appropriate for service line.

Qualifications

Education:

* Must have a Masters Degree in a behavioral health field.

* MSW is preferred.

Experience:

* Two years recent related work experience in the past three years, preferably in a  healthcare setting. * Depending on the assigned work unit/service line, work experience in a specialized field may be required (i.e. Hospice, Nephrology).

License/Certification:

* Must possess a current State Counselor Registration. State Counselor or Social Worker Certification is preferred.

* Member of the National Association of Social Workers is preferred.

* Current State Drivers License or other evidence of equivalent mobility may be required for certain positions.

* Current CPR Certification.

* Must be bilingual in Spanish and English
