* Masters Degree Required * • Provides hospital patients with psychosocial and continued care planning intervention services. • Serves as a community liaison and provides education in compliance with applicable hospital, departmental and governmental regulations and polices. • Receives referrals for patients at high risk for psychosocial/continued care planning needs from care managers and others. • Assesses patient needs and determines mode of intervention. • Implements plan of intervention preparatory to discharge in compliance with departmental and governmental regulations. • Executes plan of discharge and continued care which is mutually agreeable to patient and their family.
