Key Responsibilities Works closely with the interdisciplinary team. Arranges home care, alternate placement & facilitates access to support services. Psychosocial assessment of resident/patient needs Supportive counseling related to placement, life adjustments, family dynamics Documentation in medical chart per policy Participates in interdisciplinary care plan meetings and other meetings per supervisory discretion. Overall management of assigned caseload Requirements Bachelors Degree in Social Work or Human Services Experience in hospital/health care preferred.
